Covid Bivalent Booster Vaccine Administration

There will be a pharmacist from Walgreens Pharmacy on campus Friday, October 28th, from 8 a.m. to 12 p.m. to administer the Covid Bivalent Booster vaccine to faculty and staff. We would like to extend the offer to receive the vaccine to Davidson Day students ages 8 and up!
Parents, as student walk-ins will not be allow, please email Angie in advance to let her know if you would like for your child to get a Covid vaccine and include the completed consent form so that she can make sure they don’t miss this convenient opportunity.

If you would like for your child to get the Covid vaccine, we need your signed permission. Please print and complete this Vaccine Administration Record-Informed Consent and email it to Angie Sprayberry, School Nurse, along with a copy of your insurance card, at aspraberry@davidsonday.org.
